We are glad to welcome new patients — individuals and families, medical card holders and private patients alike. Complete the enquiry form below or phone reception and we will be in touch about registering with the practice.
How to register
- Send us the enquiry form below, or phone the practice on 074 9532445.
- We will confirm whether we can accept you and arrange a short registration visit.
- We request a summary of your medical records from your previous GP, with your consent.
- Medical card and GP visit card holders also need to change their GP with the HSE — we will help you with the form.
What to bring
- Photo ID and proof of address
- Your PPS number
- Medical card or GP visit card, if you have one
- A list of your current medication
- Details of your previous GP
- Vaccination records for children
